Our canine friends enjoy 3 daily walks in our outdoor exercise area. We are proud to offer the latest walk for dogs in Lawrence! Our last walk is at 7pm—a later walk helps your pet have a more comfortable night!

Pet 1
Please list any additional procedures (like exam, vaccinations, medication refills) your pet needs during its stay.
All pets are fed Hill’s Premium Diets. If you wish to supply your own food, please list what you are currently feeding your pet, how much and how often.
Please list any medications that need to be administered during your pet's stay, include the name of the medicine, dosage, and how often/time it should be given.
Additional Information
Pets boarding at CPAH must be current on vaccinations and free of fleas and ticks. For the health and safety of all pets, dogs must be free of intestinal parasites (negative stool check within last 12 months).
I authorize CPAH to do whatever is necessary in case of illness or an emergency situation.
